What is System of Care?
System of Care is a philosophy of how system partners can work together alongside young adults and their families to receive support and services.
The System of Care work in Erie County is a collaboration between youth, family members, providers, groups, businesses and community members.
System of Care
We believe that when we work better together, we enhance the child-serving system and help youth and their families reach their fullest potential.
The framework consists of a governance committee, a county leadership team, and several subcommittees focusing on key areas "within our strategic plan."
System of Care Principles and Goals
System of Care Erie works to partner with local providers, agencies, schools, faith-based organizations and community members to create a child and family-serving system that encourages youth and family participation and improves behavioral/mental health outcomes. While the grant assesses strengths and challenges of services for children ages birth to 21, System of Care Erie has chosen transition-aged youth as the primary target population. These are youth ages 14-26 years who have, or who are at risk for mental health challenges or other system involvement.
